All parents are given the Medical Form to complete when their child first joins the school and every year thereafter. However, if your child's medical circumstances should change throughout the year, please download, complete and return back the Medical Form to school immediately.
If your child is unwell and you would like the school to administer any prescribed medication, please complete the appropriate form and bring in to school with the medication. NO medicine can be given unless the form is completed in full and signed.
